Prescription Form for Clients

Informed Choice valued clients, please follow the instructions below to complete and send your prescription drug list securely to us for review.  We highly recommend that you take the time to complete your list as plans do change every year.

Instructions: 

• Open the fillable drug form

• Complete the form

• Save the form to a location on your computer

• Call Informed Choice customer service at 1-877-446-3676 and let them know you have a drug list to email

• Informed Choice staff will send you a secure email

• If you don’t have a Microsoft email account, click the save/download button on the attachment.  Open the attachment, and use the “one-time passcode” option to open the email (you will be emailed a code to access the attachment)

• If you have a Microsoft email account, choose the “sign-in” option

• Once in the email, reply to Informed Choice and attach your drug list
